The Admissions Process

In order to admit a child to Gateway Farm, the child must first be between the ages of 5 and 18. The legal guardian must call our office at 903-628-1112 and speak with the Program Director or Case Manager and a brief phone interview will be conducted. Based on this information, the following steps will be followed:

  • A face to face interview with both child & guardian
  • A second interview with the entire staff & psychologist
  • After both interviews are completed a determination is made on whether the child will fit into our program

 

If the child is accepted into our program, the following documentation is required before placement can be made:

  • A copy of their birth certificate
  • A copy of their social security card
  • A copy of their Medicaid or insurance card
  • School records including withdrawal and immunization
  • A current psychological evaluation
  • Medical Records
  • A current physical and TB test (within last 30 days)
